All applicants are required to read through the information provided below carefully,

1. Download the application form from the school website.
2. Complete the form as required, filling all blanks.
3. Pay a non-refundable fee Tshs 25,000 by bank deposit;

                   Acc No: 41610004291(No payment by cash will be accepted)

4. Attach the bank deposit slip (ORIGINAL COPY) with the application form and return it by post to St.Mary’s Mazinde Juu Secondary School P.O Box 90 Lushoto Tanga
5. Once the application form and the original copy of the bank slip have been received an applicant will be issued with an examination number which will be used as a passcode for the interview/Entrance examination.
6. On the day of the interview (19 March 2016) the applicant should have the following:
   

              - A copy of NECTA result slip

              - A copy of the bankslip

              - Two passport photograph (With a blue background)

              - Fully and decently dressed in her former school uniform

7. The interview is scheduled on 19^th march 2016
